        Q. What is HEALTHLYNX?
        A.HEALTHLYNX is a low-cost, high value benefits enhancement program that offers members discounts on healthcare and medical services including vision, hearing, dental care, prescription drugs, complementary and alternative medicine, vitamins, and fitness equipment. These savings are available through a nationwide network of thousands of healthcare providers and through our on-line health store.

        Q. Who is included in the HEALTHLYNX Access Card Program?
        A. You and your immediate family may use the HEALTHLYNX membership. "Immediate family" means you, your spouse and children living at your home."

        Q. Will each member of my family get his or her own card?
        A. No. Only one card is issued.

        A. No. TheHEALTHLYNX Access Card Program is not insurance, so there are none of the common exclusions or restrictions.HEALTHLYNX Access Card Program discounts and services are available to all members, regardless of age or medical condition, except where prohibited by law.

        Q. I'm getting married. Are my new husband/wife and stepchildren covered in my membership?
        A. Congratulations! As long as your new spouse and stepchildren will be living in the same household and are dependents, they can all be easily added to your face="Verdana, Arial, Helvetica, sans-serif" HEALTHLYNX Access Card at a discounted rate.

        A. Yes, absolutely. Anyone may purchase the card and use the program while they are in the United States visiting, attending school, or working.

        A. Yes. HEALTHLYNX does not keep any personal medical information or record of services that you and your family use.

        Q. What is HEALTHLYNX Refund/Cancellation Policy?
        A. You have the right to cancel this plan within 30 days of the effective date for a full refund of fees paid.

        Q. How do I get my HEALTHLYNX Access Program discounts?
        A. Within two (2) weeks of receiving your paperwork, you'll receive your complete membership kit, including the HEALTHLYNX Access Card, a partial list of participating providers in your area and complete details on each HEALTHLYNX program. Simply present your HEALTHLYNX Access Card to the participating provider before services are rendered. Your discount will be applied on the spot when you pay your bill.

        Q. How do I find a vision, hearing, dental or other healthcare provider in my area?
        A. That's easy. You can search on-line, by visiting our on-line Provider Search Directory at www.healthlynx.com. Just type in your zip code and we'll list some providers closest to you. You can also find out if a particular provider is part of our network by calling HEALTHLYNX Customer Service toll-free during business hours Monday through Friday 8AM - 8PM EST at (800) 308-0374. Your Membership Kit will include a partial list of the participating providers closest to you.

        Q. I went to one of the healthcare providers on my list and they would not accept my membership card. What do I do?
        A. First, please accept our apology. Each provider in the HEALTHLYNX Access Card Program has a contract with our provider network. It's possible that the person you spoke with either made a mistake, or was unaware of this network. Just call HEALTHLYNX Customer Service toll-free during business hours Monday through Friday 8AM - 8PM EST at (800) 308-0374. We'll be happy to contact the provider for you and straighten out the problem.

        Q. What dental care discounts are available through the HEALTHLYNX Access Program?
        A. Most dental care procedures are discounted, including exams, routine cleanings, fillings, extractions, crowns, dentures, and other preventive and diagnostic services. Discounts also are available on periodontal, orthodontic and other specialty procedures in most markets. Your savings on each treatment will be 10% to 50% off the participating dentist's usual fees.

        Q. What if I need a dentist while I'm out of town?
        A. Your membership card is valid in most states. Just call HEALTHLYNX Customer Service toll-free during business hours Monday through Friday 8AM - 8PM EST at (800) 308-0374 for the names of the providers closest to the area in which you're staying or you can search our on-line Provider Directory.

        Q. What does the vision benefit cover through theHEALTHLYNX Access Program?
        A. You can save 10% to 60% on the national average price of prescription eyeglasses, contact lenses and other specialty items through our network of over 12,000 participating retail optical outlets nationwide. Laser Surgery is also available, in many areas. Non-prescription sunglasses and disposable contact lenses are available through our mail order service. By presenting your HEALTHLYNX Access Card, you will automatically be charged the discounted rate.

        Q. What does the hearing benefit cover through the HEALTHLYNX Access Program?
        A. This benefit enables you to receive a free consultation from an audiologist in one of the largest networks in the country. You also can receive discounts up to 20% on hearing aids, of 10% - 20% on programmable/digital hearing aids, hearing aid repairs as well as related hearing products, excluding batteries. Also, receive free annual cleanings and check-ups of hearing aids purchased through our program.

        Q. How do I use the retail pharmacy benefit?
        A. Simply present your HEALTHLYNX Access Card when you go to pick up your prescription at a participating pharmacy. In fact, the pharmacy you're already going to may be one of the 40000 pharmacies participating in the program nationwide. Your savings - upto 30% on both brand name and generic drugs - will be applied automatically to your bill when you pick up your prescription.

        Q. How do I use the mail-order pharmacy benefit?
        A. When you are ready to renew your prescription, call HEALTHLYNX Customer Service at (800) 308-0374 to obtain a prescription mail order form. Next, complete the Prescription Mail Order Form, and have a copy of your prescription handy while you call our Mail Order Prescription Program toll-free at (800) 396-2219 to get a price quote. Then mail your completed mail order form, your prescription, and a check for the amount or credit card information to: Sav-RX Prescription Service, P. O. Box 8, Freemont, NE 68026-0008. You'll typically receive your prescription within 5 to 7 days after we receive your order.

        A. At theHEALTHLYNX home page, click on "Store". Where indicated, click to be automatically directed to the Health Store provided through our parent organization BrainyBrawn, Inc. Please remember, to enter your HEALTHLYNX Access Card number at checkout to receive your additional discount.
